Student Expectations for the Athletic Training Room
Athletic Medical Participation Form Requirements
Prior to athletic participation, families are to submit an annually completed medical history with physical. And, for each school year, families will complete participation information that provides contact information and acknowledgement of rules.

Participation information and medical history can be submitted electronically at https://northeastisd.rankonesport.com A larger screen, such as a tablet, laptop, or desktop, to complete the information is easier to use than a phone. Instructions to complete the on-line forms is on the website as well as a PDF linked here.
When using this feature, in the uppermost fields, please only use the student's birthday and student's NEISD ID number. For ID numbers that begin with zero, do not enter the beginning zero. This should cause certain portions of the form to auto-fill. As you complete the form be sure that each box, field line has a response even if the response is “N/A”.

Student Accident Insurance Information
North East Independent School District will make available the Student Accident Insurance Plan for the convenience of our students.
Participation in this program is strictly voluntary. In accordance with the Texas Tort Claims Act, please be reminded the District cannot be responsible for costs of treating injuries or assume liability for any other costs associated with an injury at school or any school related function unless the personal injury is caused by a district employee’s negligent operation of a motor vehicle while performing district duties.

- Students are to report for routine (non-emergency or non-urgent) evaluation, treatment, or rehabilitation of school athletic injuries during available non-academic time. Students will be seen during athletic times only with the agreement of the Athletic Trainer and Coach; out-of-season athletes may be referred to non-athletic period times.
- Students seeing a healthcare professional for any reason are to bring a written note to the Athletic Trainers indicating the condition being treated and restrictions to athletic participation, if any.
- Students are to wear appropriate apparel as well as shower after activity. Appropriate apparel during athletic periods, practices, or games is the team’s practice or game uniform. Appropriate apparel during non-athletic, non-academic times are to conform to school requirements and allow for effective evaluation, treatment, or rehabilitation (i.e. shorts for a knee injury).
- Students not engaged in consultation, evaluation, treatment, or rehabilitation are to remain outside the Athletic Training Room.
